Health insurance is not hard to get here, you just have to be a resident to get some policies. Others can be had that are international as well. My wife and I have both a small medical plan by Humana and a catastrophic plan. Not sure I can answer the question of “best” city and many may claim their own to be, but I have heard good things about the hospitals in Cuenca, Quito, Manta, Salinas, Guayaquil so most of the larger cities. We have a new hospital on the coast where we live but have not had to use it yet.
